In order to serve you efficiently, we ask that you bring to your appointment:

• Insurance cards
• Photo ID (to comply with the Federal Trade Commission Red Flag rules)
• List of current medications, including the dosage

Please arrive 15 minutes early to your scheduled appointment time.
