Dear New Patient,
Thank you for entrusting us with your medical care. Our providers and staff are committed to medical excellence in a compassionate environment. We measure our success by our patient’s satisfaction and exceptional medical outcomes.
In order to serve you efficiently, we ask that you bring to your appointment:
- Insurance cards
- Photo ID (to comply with the Federal Trade Commission Red Flag rules)
- List of current medications, including the dosage
Please make sure to have your new patient paperwork turned into our office 24 hours prior to your appointment. Our office requires that we have your paperwork in advance otherwise we may have to cancel and/or reschedule your appointment. We do this so that our providers and nurses will have a chance to review your medical history and be prepared for your visit. On your appointment day please arrive 30 minutes early to your scheduled appointment time.
