Patients visiting from out-of-town

Unfortunately, illnesses or injuries won’t wait until you are home to see your regular physician.  The providers at Family Health Center are happy to see our out-of-town visitors and bill their insurance plans; however, new patients are expected to pay for the visit at the time of service.  You will be refunded the amount paid by your carrier once your insurance responds.

Patients with Private Insurance, Medicare, or Medicaid

Family Health Center is pleased to offer third-party billing as a service to our patients.  Our patients also have certain responsibilities in order for our account representatives to bill for your visit accurately and timely.  Patients must have a valid copy of their insurance card at the time of service, and your co-payment is required at time of service.  This is a contractual obligation you, the insured, agreed to when you signed up for your plan. Patients who request that we bill them for their co-payment will also be responsible for a $10.00 service charge. Medicaid patients must have a valid card, not a pending claim, at check-in.  Medicare patients must have a valid card at check-in.  Please present your card to the receptionist at the time of your visit.

Patients without Insurance

We at Family Health Center realize that there is a growing population of patients who do not have insurance, and we try to accommodate those patients as much as possible.  To that end, while full payment is expected at the time of service, we do offer discounts on certain services.  If you do not think you can meet your financial obligation at the time of service, you are invited to speak with an accounts representative to arrange payment.  Patients in good standing may skip a monthly payment provided that you give us the courtesy of notification and a subsequent “catch-up” payment the following month.  Patients who have not fulfilled financial obligations of prior arrangements will be required to make a payment before making another appointment.

Patients Referred to Collections

Patients who have been referred to collections are required to make a minimum payment of $50.00 on account prior to seeing the physician.  An exception to this policy is if you present a current Medicaid card.

Motor Vehicle Accidents

Visits for a motor vehicle accident must be paid for at the time of service.  Billing to attorneys is not a substitute.  The accounts representative is happy to provide the patient with a copy of the bill for their records and/or for their own billing.
